Cory Monteith died unexpectedly at the age of 31 in his Vancouver hotel room on July 13 as a result of what has been confirmed as a lethal mixture of heroin and alcohol.

At the scene of Monteith’s death, investigators found a spoon with drug residue and a used hypodermic needle, according to the British Columbia Coroners Service. Two empty bottles of champagne were also located in the room.

Monteith had checked into the hotel on July 6 and was scheduled to be heading out on July 13. When the Glee actor missed his checkout time, hotel staff found him unconscious on the floor. “There were others with Mr. Monteith in his room earlier last night, but video and fob key entries show him returning to his room by himself in the early morning hours and we believe he was alone when he died,” acting Vancouver Police Chief Doug LePard said in a statement.

Throughout his career, Monteith was open about his struggles with drug abuse in his teens. In April of this year, the actor voluntarily checked himself into rehab. At the time, his girlfriend and fellow Glee star Lea Michele voiced her support, saying in a statement, “I love and support Cory and will stand by him through this. I am grateful and proud he made this decision.”

In the aftermath of Monteith’s shocking death, friends and colleagues of the actor expressed their devastation at the loss of the young star. Jane Lynch, speaking on behalf of all those who knew Monteith, delivered an emotional eulogy at this year's Emmy Awards. “If you were lucky enough to know Cory as we did, and witness firsthand Cory’s goofy, breezy sense of humor, his natural instinct for inclusiveness and his unbridled sense of generosity day in and day out,” Lynch said. “I promise, you’d have loved him even more.”

The Glee cast filmed a tribute episode for Monteith and his character Finn Hudson for the Fox musical dramedy that will air Thursday, Oct. 10.
